VA COMMUNITY CARE
HOW VETERANS CAN RECEIVE CARE AT PREMIER
Eligible Veterans may be able to receive community-based healthcare from participating providers when authorized through the VA Community Care program.
STEP 1:
Contact Your VA Healthcare Team
Speak with your VA primary care provider or appropriate VA healthcare team about your chiropractic, physical therapy, rehabilitation or musculoskeletal healthcare needs.
STEP 2:
Request Community Care
Ask whether you are eligible to receive the recommended service through VA Community Care.
STEP 3:
Request Premier Total Healthcare
Let your VA healthcare team know you would like to receive authorized care at Premier Total Healthcare in Hollywood, Florida.
STEP 4:
Authorization & Scheduling
Once the appropriate VA authorization or referral has been completed, contact Premier so our team can assist with scheduling and the next steps.
STEP 5:
Begin Your Care
Your Premier provider will evaluate your condition and establish an appropriate chiropractic, physical therapy and/or rehabilitation treatment plan based on the services authorized by the VA and your individual needs.

IMPORTANT VA COMMUNITY CARE INFORMATION
VA Community Care eligibility, referrals, authorizations, covered services and treatment frequency for chiropractic care, physical therapy and other authorized services are determined by the U.S. Department of Veterans Affairs and the applicable VA Community Care authorization.
Being treated by a community provider does not mean that every service offered by that provider is automatically covered or authorized by the VA.
Veterans should confirm eligibility and obtain any required VA referral or authorization before receiving services they expect to be covered through VA Community Care.
Premier Total Healthcare can assist patients with questions regarding scheduling after the appropriate authorization has been received.
